A valid passport and visa are required for travel to India.
A Medical Visa is given to those seeking medical treatment in reputed/recognized specialized hospitals/treatment centers in India. Up to two attendants who are blood relatives are allowed to accompany the applicant under separate Medical Attendant visas.

Actual, original signed passport and one photocopy of passport information and signature page.The passport must be signed, valid for 6 months beyond stay in India, with at least two blank passport pages available for India visa stamp. Amendment pages in the back of the passport are not suitable for Indian visas.

Please Note: The actual traveler must sign their own visa application form, and signatures on the application must match the signature in the applicant's passport.
The Indian visa application form must be printed on two pages, and requires a signature on both pages. The applicant must sign under the photo on the first page, and at the bottom of the second page. If the signatures on the application do not match the signature on the passport, a new original application form will be required.

One recent 2 x 2 passport type photograph, in color, front view and with a plain/white background. Glasses should not be worn in these photos; photos should not be stapled to the application.

This form must be printed out, completed and signed by all visa applicants.

Proof of Residential Address Clear photocopy of applicant's proof
of residence/address that matches the present address on the visa
application exactly. Photocopy of state issued ID (Driver License or ID),
or most recent major utility bill (Water, Gas, Electric, Sewage) or a copy
of a valid/current lease containing both the landlord and tenant
signatures. If applying in San Francisco a copy of the state
ID or Drivers License must also be included when submitting a utility bill
as proof of address.
Parental Authorization For Minors
Note: In Chicago, Houston, San Francisco and Washington DC this form must be original and must be notarized.
Copy of each parents' bank statements
Note: This is required in San Francisco only. Parents' joint account statements are acceptable when showing both parents' names. If the minor is traveling alone, parents' past three bank statements must be provided.
Copy of the minor's birth certificate.
Copy of both parents' signed passports.
Minors Signing the Application Form: A minor child should sign their name inside the signature box under the photo on first page of the application. If the child is too young to sign their name, the child's thumbprint must be stamped inside the signature box under the photo on first page of the application. On the second page of the application, the minor child should not sign the application. On the second page of the application, both of the child's parents must sign the application, and both parents' signatures should be notarized. Applications without both parents' notarized signatures will not be accepted.
The visa application must be signed by both parents and notarized. A copy of a parent's driver's license or utility bill can be used for proof of residential address in the US.
Minors of Indian Origin: a minor must submit a copy of the parent's cancelled Indian Passport and a copy of the parent’s Renunication Certificate. If they have a copy of the cancelled India Passport they may complete a Renunciation certificate and submit it. If they do not have either document, they will need to complete and submit the required documentation for a Deemed Surrender Certificate.
A certificate of preliminary medical advice from the country of origin/country of residence that shows the applicant has been advised to go for specialized medical treatment. In cases where the applicant desires to go for treatment under the Indian system of Medicine, the case may also be considered.
Residents in San Francisco jurisdiction: Applicants with occupation of Student, Retired or Unemployed; three most recent bank statements are recommended.
For couples seeking surrogacy in India, the following conditions need to be followed for Medical Visa and Medical Attendant Visa for surrogacy purposes:
- A copy of marriage certificate showing the foreign man and woman have been married for at least two years.
- A letter from the Embassy of the Foreign country in India or the Foreign Ministry of the country to which the applicant couple belongs stating clearly that (a) applicant couple's country recognizes Surrogacy, and (b) the child/children to be born to the commissioning couple through the Indian surrogate mother would be permitted entry into the applicant couple's country as biological child/children of the couple commissioning surrogacy.
- An undertaking letter stating that the applicant couple would take care of the child/children born through surrogacy.
- Documentation demonstrating that treatment will be done only at one of the registered Assisted Reproductive Technology (ART) clinics recognized by Indian Council of Medical Research(ICMR).
- A notarized agreement between the applicant couple and the prospective Indian surrogate mother.
